Description

The Italian Program C4D-Italia is part of the 3-year EU Joint Action CARE4DIABETES (C4D), funded under the EU4HEALTH program.

C4D deals with a multidisciplinary lifestyle treatment intervention for type 2 diabetes based on the EU Best Practice ReverseDiabetes2Now. The C4D protocol (https://doi.org/10.1186/s12889-025-22246-w) aims at evaluating several aspects of the intervention and integrates nutrition, physical activity, sleep, and stress management.

In Italy, the Italian National Institute of Health involved three Clinical Centers (Pisa University Hospital; ASL ROMA 2 (Rome); Agostino Gemelli University Hospital Foundation (Rome)) to implement the Pilot named C4D-Italia.

Besides traditional in presence meetings, C4D-Italia program successfully relied on continuous online interaction among patients and the multidisciplinary team, supported by an ad-hoc digital platform and cross-media tools. The zip file “C4D-Italia MATERIALI DEL PROGRAMMA” contains relevant educational and practical materials – all in Italian - developed and delivered during the program.

Materials are organized into 7 sections, respectively dealing with: program objectives; physiopathology of diabetes; practical guides (decalogues); and insights into clinical management, nutrition, physical exercise, stress management and sleep hygiene.
